| Description | Bayshore Home Health specializes in the areas of community health care by providing quality home health care and supplemental staffing. Services include: * In-home nursing and homemaking services * Nursing and Personal Support institutional staffing relief * Services to help reduce hospital stays and maintain the elderly in their homes * In home nursing provides skilled nursing care to patients of all ages , including palliative care, pediatric care, wound care, mental health care, post-operative care, medication management * Institutional Private Duty Nursing * Enterostomal therapist- wound and stoma therapy advice * Nurse Continence Advisor services * Flu shot clinics * Personal Support Workers certified in care for clients with Acquired Brain Injury * Personal support workers and Health Care Aides provide a wide range of care and assistance including light housekeeping, meal preparation, personal care * Companions provide friendly visiting, companionship and accompaniment to appointments * Respite care is offered to families of Alzheimer patients, developmentally handicapped, severely disabled and frail elderly * Special short visits include bath service, home support visits, nursing visits. Live- ins (for long or short term) * Home clinics also available for housebound people * Bayshore also provides health teaching on alcohol use/abuse and associated health risks * Specialized foot care nurses who provide in home service on an individual basis * Nursing Clinic: open Monday to Sunday 9am-8pm, by appointment booking and with doctor's orders. Plenty of parking available. Wheel chair accessible * Bayshore staff assist with assessing and processing of health insurance claims for their clients * Mask Fit Testing * Guest speakers provide information on convalescent care and services * In-home Nursing service provides ongoing at-home health management to seniors and people of all ages * service includes: physical assessment, medication review, nutrition review, skin health screening, mental health assessment, infection screening, mobility assessment, visit reportsFalls Prevention * Fall prevention assessments conducted in the client's home are offered to identify any potential / actual risks for falls * Education related to falls prevention, medication review and education, and access to related resources (such as assistive devices, medical assessments from nurse practitioners) also provided at time of assessment * Bayshore Home Health is a member of the Staying Independent Falls Prevention Coalition (SIFPC) | |||
