| Description | Provides assessment, education, support and respite for caregivers as well as a standardized care plan for the individual with Alzheimer Disease and dementia * specially trained Respite Care-workers provide services to clients on a home visiting basis under the supervision of a Program Coordinator * caregivers utilizing the service will benefit from time away from care-giving, providing them with rest, and time to pursue social activities, etc
