| Description | Assists individuals with an acquired brain injury or spinal cord injury to re-establish their lives in their own homes and communities * facilitates and supports the implementation of ongoing, community based rehabilitation and personal care in a functional context, up to 24 hours per day 7 days per week * provides one-to-one assistance necessary for supported independent living, and develops and implements strategies and/or supports that facilitate active community participation * services and supports include: Life skills coaching, Job coaching, Cognitive & Behavioural interventions, Recreation and Leisure participation, Academic/Educational Assistance & Tutoring, Family Education & Counselling, Personal Care and Housekeeping * offers "Linking People and Places" Travel Support/Attendant program for seniors and people with disabilities to assist in travel throughout Canada and US |
