| Description | Provides an access point to health and personal support services to help individuals live independently at home or to assist them in making the transition to a long-term care home * also provides information about, or links individuals to, services available in the community Services: * Information about availability and the methods to access health and community support services that are directly related to the needs of the individual * Client Care Coordinators who are specially prepared to address the community care needs of individuals with a variety of care needs * Health and functional assessments of the individuals in their home by regulated health care professionals and coordination of services that are available under MOHLTC funding * Referral to appropriate services and volunteer organizations * Access to supplies and equipment and funding to modify living space to accommodate limitations * Client Care Coordination on-going assessment and monitoring as needed * Coordinates admissions to Long Term Care Homes, including Short Term Care in these facilities (eg for those recovering from illness or surgery) * One of 14 CCACs in Ontario working together in the community to enhance access to community-based health services for people across the province * work closely with community health care partners including the Local Health Integration Network (LHIN), physicians, hospitals, Family Health Teams, health care agencies and community support services to ensure clients receive the needed community-based health services Member of the North Simcoe Muskoka Community Support Services Coalition | |||
