| Description | Connects individuals with a wide range of services including case management, information and referral, professional health services and supportive care services * see list below for the many areas of healthcare, home health care, and health service providers NSM CCAC works with in your community to connect you to the best care for your situation: In Home Services Nursing Physiotherapy Occupational Therapy Speech and Language Pathology Social Work Nutritional Services Respiratory Therapy Personal Support Medical Supplies Equipment Rentals Adult Day Services Residential Hospice Long Term Care Admissions Physiotherapy component of CCAC services are available to those unable to access services outside the home due to their condition * teaches safe ways to maintain mobility and activity level as well as ways to prevent falls * assists school staff to carry out appropriate exercises and gym programs with a child at school * provides treatment following a fracture or joint replacement Member of the North Simcoe Muskoka Community Support Services Coalition | |||
