Social Work component of CCAC services includes the following: 
* in crisis situations, provides housing and community support information

In order to receive services from the CCAC, client must have  
* a valid Ontario health card  
* a need that cannot be met on an outpatient basis  
* a medical condition that can be adequately treated in the home  
* a need for at least one health care professional or personal support service
