| Description | 12 week health and wellness program for people 55 years and older who are frail and living on their own in the North Simcoe Muskoka LHIN and who may have a history of and/or fear of falling * program designed to help seniors maintain their independence * volunteers go to clients' homes once a week and assist with 15 gentle exercises, including walking * all volunteers participate in a 2-day training session * group classes are also available at certain locations Falls Prevention: Exercise programs are designed to increase strength and balance and reduce the risk of a fall * a mini falls risk assessment and home hazard assessment are also offered VON Canada Simcoe County Branch is a member of the Staying Independent Falls Prevention Coalition (SIFPC) | |||
