Description12 week health and wellness program for people 55 years and older who are frail and living on their own in the North Simcoe Muskoka LHIN and who may have a history of and/or fear of falling * program designed to help seniors maintain their independence * volunteers go to clients' homes once a week and assist with 15 gentle exercises, including walking * all volunteers participate in a 2-day training session * group classes are also available at certain locations

Falls Prevention:

Exercise programs are designed to increase strength and balance and reduce the risk of a fall * a mini falls risk assessment and home hazard assessment are also offered

Residents of North Simcoe Muskoka Local Health Integration Network, 55 yrs and older; frail, elderly, housebound, have a history of falls, at risk for falls, or have a fear of falling
ApplicationReferral - self * referral from caregiver, CCAC, physician,nurse practitioner, physiotherapist, Veterans Affairs Canada, hospital, etc.
FeesNone - For participants * Program fee of $4 per session has been sponsored by the North Simcoe Muskoka LHIN
