DescriptionHelps make the transition from hospital to home * provides transportation and the services of a HAL Attendant * once at home the HAL Attendant can provide assistance with groceries, medication and medical supplies pick up, light housekeeping, laundry and meal preparation * the HAL Attendant will provide the client with a package of community support services * followup within 48 hours by HAL supervisor
