| Description | An Ontario Government hotline will help match people with a doctor in their area that has an open practice and is willing to take on new patients. Callers will be referred to a family health care provider as soon as one becomes available. There is no time guarantee. Telehealth Ontario staff will take down a health history and pass it on to a nurse. Priority is given to people with greater health needs. To be on the list, a patient cannot still be registered with a family doctor anywhere in the province. If they are registered they must deroster with their current doctor, then call Health Care Connect to apply. To cancel enrollment or de-roster with current family physician, contact their current health care provider indicating that they wish to end enrollment OR telephone ServiceOntario at 1-866-532-3161. | |||
