| Description | The following programs are offered at this facility: LifeLabs Specimen Collection Services provide: * Mobile specimen collection services on Tuesday and Thursday mornings from 7:00 am to 9:00 am. A laboratory requisition form a physician is required. Appointments are not needed. For more information about LifeLabs, please visit their website at www.lifelabs.com or call 1-877-849-3637. North Simcoe Family Health Team (NSFHT) provides: * a mobile clinic on Thursdays. Appointments are required. To book an appointment, please call the Springwater clinic on Thursdays at 705-322-9099 or the main NSFHT Midland office at 705-526-7804. NSFHT services are provided by Physicians, Nurse Practitioners, Registered Nurses, Social Workers, Dieticians, and Diabetes Educators who share patients within the NSFHT. Patients without a family physician my enroll with the Springwater Health Services Centre by contacting them. LifeLabs Specimen Collection Services are open to anyone with a test requisition from their any physician. | |||
